Innovation, technology, workforce and medicines optimisation - lessons for 'A Healthier Future' from our Care Homes Vanguard

Showcasing the work of our Care Homes Vanguard, this event will allow delegates to learn from our successes, benefit from lessons learned and take away practical ideas to integrate Vanguard best practice into their own improvement programmes.

The format of the showcase is:

  • A national overview on Vanguard learning, from William Roberts, National Lead, Enhanced Health in Care Homes, New Care Models Programme

Four themed workshops:

  1. Improving hospital discharge processes - the role of the ‘Impartial Assessor’ and the care home ‘Red Bag’ scheme
  2. Multi-disciplinary working - Medicine Optimisation in care homes and the East of England Ambulance Service NHS Trust’s Early Intervention Vehicle
  3. Technology in care homes - NHS.net and real-time GP access to patient records
  4. Workforce development – up-skilling care home staff to prevent avoidable hospital admissions.

East and North Hertfordshire Clinical Commissioning Group (CCG) is the organisation responsible for commissioning (planning, designing and paying for) your NHS services. The CCG is made up of local GPs and health professionals, working together with other clinicians and patients, to decide how the local NHS budget of £724m should be spent.

The CCG serves about 600,000 people registered at 57 GP Practices across east and north Hertfordshire. Our GPs understand the health needs of their patients, and we believe this local approach to commissioning helps ensure good quality services that meet the needs of the population.
